The Aetna Critical Illness Plan can help you protect your finances if you are diagnosed with a covered condition. The plan pays cash benefits to you when you are diagnosed with a serious condition like:

 

  • Heart attack or stroke
  • End stage renal failure
  • Cancer and more

 

You are guaranteed to be accepted into the plan with no medical questions. No pre-existing conditions are excluded. Please note: your diagnosis must occur after the coverage effective date to be eligible for benefits.

 

You can use the money to help cover your deductible or everyday expenses like utility bills, mortgage payments and groceries. It’s up to you. See the attached benefit summary for details of coverage, including limitations and exclusions.

  • How do I file a claim?

    Go to myaetnasupplemental.com and either “Log In” or “Register”, depending on if you’ve set up your account. Click the “Create a new claim” button and answer a few quick questions. You can even save your claim to finish later. You can also print/mail in form(s) to: Aetna Voluntary Plans, PO Box 14079, Lexington, KY 40512- 4079, or you can ask us to mail you a printed form.

     

    If you require language assistance, please call Member Services at 1-800-607-3366 and an Aetna representative will connect you with an interpreter. If you’re deaf or hard of hearing, use your TTY and dial 711 for the Telecommunications Relay Service. Once connected, please enter or provide the Aetna telephone number you’re calling.

  • How can this help me?

    The Aetna Critical Illness Plan can help you protect your finances if you are diagnosed with a covered serious condition. You can use the money to help cover your deductible or for everyday expenses like utility bills, mortgage payments and groceries. It puts extra money in your pocket so you can focus on recovery.
  • When can I enroll?

    All eligible employees can enroll during annual enrollment. New hires can enroll during their new hire enrollment period. You will not be able to enroll outside of your new hire enrollment period or annual enrollment unless you experience a qualifying life event, like marriage, birth of a child, etc.

     

  • What if my employment status changes?

    When you leave or retire from your current employer, you can continue your coverage without interruption, subject to applicable law and the plans' terms and conditions. Although payroll deduction will no longer be available, you can pay Aetna directly. Portability forms are available at www.myaetnasupplemental.com or call Member Services at 1-800-607-3366 for assistance.
  • Who is eligible?

    You can sign up for coverage for yourself and your eligible dependents during your new hire enrollment eligibility period, during the annual enrollment period or if you experience a qualifying life event. No medical questions are asked. The plan is designed to complement, not replace, major medical insurance.
  • Will my rates increase as I get older or if I file a claim?

    Your rates will not increase due to health or individual claims. Your rates are based on 5-year age bands and will go up at the beginning of the year that you move into a new age band. Employees and spouses are no longer eligible when they reach the age of 80 years old.
  • Will I have to take a medical exam to get this coverage?

    No! This coverage is guaranteed-issue (which means you may qualify for coverage without having to submit to a medical exam or answer health questions). Please note: your diagnosis must occur on or after your coverage effective date for benefits to be paid.
  • What types of critical illnesses am I covered for?

    The Aetna Critical Illness Plan is a voluntary plan that can help you protect your finances if you are diagnosed with a covered serious condition, like heart attack, stroke, end stage renal failure, cancer and more. The plan is designed to complement, not replace major medical insurance. Medical plans help pay providers for services and treatment. The Critical Illness Plan pays cash benefits to you, no matter what is covered by your medical plan or any other insurance. 

    The plan pays up to $10,000 for diagnosis of the following conditions:

     

    • Heart Attack (Myocardial Infarction) 100%
    • Stroke 100%
    • Coronary Artery Condition Requiring Bypass Surgery 25%
    • Major Organ Failure 100%
    • End-Stage Renal Failure 100%
    • Paralysis 100%
    • Loss of Sight (Blindness) 100%
    • Loss of Speech 100%
    • Loss of Hearing 100%
    • Occupational HIV 100%
    • Coma 100%
    • Benign Brain Tumor 100%
    • Third-Degree Burns 100%
    • Alzheimer’s Disease 25%
    • Parkinson’s Disease 25%
    • Lupus 25%
    • Multiple Sclerosis 25%
    • Muscular Dystrophy 25%

     

    Your plan includes subsequent diagnosis and recurrent diagnosis benefits. It also includes an annual health screening benefit for a covered preventive test.
